- Log in to Zapier
- Access your dashboard
- Create a new Zap
- Click “Make a Zap”
- Set Cognito Forms as the trigger
- Choose “Cognito Forms → New Entry”
- Connect and select your form
- Add Twilio as the action
- Select “Send SMS”
- Connect your Twilio account
- Configure SMS
- Choose your Twilio “From” number
- Map “To” to the form phone field or a constant
- Craft the message using form fields
- Test the Zap
- Submit a sample entry, verify SMS receipt
- Activate the Zap
- Turn on the Zap for live SMS alerts

- Use the Create a Webhook/Choose a hook
- Connect and select your form
- Add Twilio as the action

- Connect Twilio with your Account SID and Auth Token
- Configure SMS
- Set “From” to your Twilio number
- Map “To” to a form phone-number field or a fixed number

- Test the scenario
